Completing the orders is fairly simple and straight forward. A simple way to think about the orders is you are essentially pre-filling out the case information and the judge will decide which one to use depending upon the outcome of your sealing efforts.
You'll want to have a few pieces of information available before completing them:
- Case #
- Arrest #
- Court address
- Where the case was filed (county, district, municipal/city)
Below you will find instructions for completing each different type of order.
The order to seal conviction records is pretty straight forward. Some of the information requirements are different depending upon whether you're sealing one case, multiple cases, or municipal cases. This section is broken down into two sets of instructions. First, instructions for sealing a single case(or single municipal case). Then, instructions for sealing multiple cases.
Instructions for sealing a single case(or single municipal case):
Sealing a single case:
If you are sealing a single case, you'll want to fill out the following sections:
Court: District/County/Municipal
Court mailing address
Defendant name
Case number(this will be the case number of your offenses)
Defendant information(remember to use permanent information here)
Sealing multiple cases(filed in county/district court)
County/District Court
Court mailing address
Case number will be provided by the court
Background information(defendant name, address, etc.)
The order denying request to seal conviction records is similar for a single conviction sealing, municipal conviction sealing, and for sealing multiple cases. There will be some differences in what is filled out depending upon whether you are sealing a single conviction record or multiple conviction records.

If you are sealing a single case, you'll want to fill out the following sections:
Court: Likely county court or municipal court
Court mailing address
Defendant name
Case number(this will be the case number of your offenses)
Sealing multiple cases(filed in county/district court)

Court mailing address
Case number will be provided by the court
The order and notice of hearing to seal conviction records is the same for single conviction sealing, sealing a municipal conviction, and for sealing multiple cases though there will be some differences in what is filled out depending upon whether you are sealing a single conviction record or multiple conviction records.

If you are sealing a single case, you'll want to fill out the following sections:

The documents for sealing municipal convictions are very similar to the documents for sealing a single case or multiple cases. The one difference is where the petition is filed. This will determine what information is provided. Below, you'll find directions that do not change across the forms and the two differences based on if you file in the municipal court or the district court.
Information that doesn't change regardless of where you file:
Defendant name
Background
- Defendants full name
- Defendants date of birth
- Defendant mailing address
Information that does change based on where you file.
Court check box (municipal or district)
Court mailing address (city/town or county)
Case number:
- If you are filing in municipal court you will use the case number of the conviction
- If you file in district court the case number will be provided by the court
